In the timeline, there's a rumor of someone in Season 11 blowing a fully-revealed Bonus Round puzzle.
If that turns out to be true, nothing will compare to that.

^Wow, that would be crazy if true. Though of course it has happened in the main game before with REGIS PHILBIN & KELLY RIPA. The infamous CORNER CURIO CABINET puzzle had everything but the B filled in also.
Looking at the season 11 timeline, EXAM was also not solved despite only the X missing.

I'm probably late on the draw to this, but I'm putting a stop to this now, and here's why.
I don't think there's any question that most of you reading these boards are above-average puzzle solvers, and that's fine.
But it's a really great way to turn people off from watching the show if you keep telling them "I can't believe that wasn't solved, it was so easy" or something to that effect. Calling attention to that, as someone mentioned upthread as being the intent of this thread, actually serves to make contestants feel bad about their performance (whether or not they read these boards--but let's assume that they do). It'll make them less likely to come here and talk about their experience on the show because of how awfully some members treated their performance when they didn't know they were reading.
You're all better than this.
