This is probably a stupid question, but I'd like to know, has there ever been a Sudden Death question on Family Feud where the family that answered it correctly DIDN'T get enough points for the win?

I'm going to take the educated guess and say no.
They make #1 really obvious for 80+ points in this situation, and it would be very hard to be under 60 points after a double/triple round without >300 for other team

I can't give a date or even a year on this one, but I'm actually pretty sure I saw an episode a long time ago (either Karn or O'Hurley) where it DID happen. In that case, they just did another Sudden Death question.
Also, unless things have changed a lot recently, #1 on the sudden death isn't as high as 80, it's usually 55-65, 70 at the absolute highest. So it's possible that a family could have as much as 100-120 and still not make 300 (rare, but possible).

It was late in O'Hurley's final season, I do believe.

Someone who went to a recent taping said that the Triple question was scrapped near the end because second place had 0 and the amount on the board clearly meant Sudden Death was coming. The "official" excuse was that they didn't want to show favoritism, but it's pretty clear that they didn't feel like programming a sixth question.

Well, I'm pretty certain that I did see such an episode at one point (although like I said, I'm unsure of when). It's possible that maybe the episode I recall was in part responsible for the change to the current policy.
It's a rare enough occurrence for it even to happen in the first place, but given the need to stick to increasingly tight timing restraints when taping, I can understand the need to avoid a repeat of the event.

Yeah, Indonesia had one. Indonesia using the old way tiebraker (The 1 strike method instead the 1 answer method) The score was 289-0 and turns into 289-297 in a tie-braker round. A nice episode. I remember that every single second at that epsode is cut. So you'll see that the host is "teleporting" Since every second that consist the host walking to the next player is being cut.
