This was a question that came up in a casual conversation that made for interesting speculation and I was wondering if any former players or members have knowledge on what actually happens. What's the show's procedure, if known, if someone has to drop out in the middle of a game (ex: medical emergency)? Do they just start a new game with an alternate even if Round 4 is starting? Do they leave the podium empty and just let the two remaining play or add an alternate who unfortunately gets a short appearance?
I figured there would be a procedure in place since there could also be other situations along those lines; like maybe having to disqualify someone for unsportsmanlike conduct or cheating or another serious rule violation and how they might handle that. How would those be potentially handled also?

I'm pretty sure that that's what they do. They have two contestants, usually locals, as stand-by contestants. An on-stage compliance official monitors the action, and if there are any violations (or anything suspicious), she would alert a producer. As to what would happen from there, I don't know. It is a federal offense to cheat, rig, or deceive any game show. If there is a medical emergency, and the contestant is unable to continue, I think the game might be scrapped and restarted with the alternate.

If that's how it goes, talk about bad luck if one player had say $30k+ going into round 4 by winning the car and hitting express, then having to start over because someone unfortunately for example had a heart attack as the 3rd toss up as going.

The thing is, on Wheel all contestants win whatever they solve with (barring a few exceptions), so scrapping an episode might mean the players don't get what they won the first time and have to settle for what they won the second time...which could be quite a bit less. If that is the case, though, then a losing contestant with enough spite could make themselves sick and force the game to restart.
For the record, I honestly don't know how the show would handle an unairable episode since as far as I know it's never happened. It could in fact be that, if an episode does have to be scrapped, the players get what they originally won in addition to whatever is won in the remount.
Alternately, it could be easier to simply replace the sick contestant with a relative from the audience, explain what happened (even if it's mid-round; do a mid-round commercial break if absolutely necessary), then move on. I know The Price Is Right did this in recent years...although in this case it'd probably render the relative ineligible.
Despite all this, two questions come to mind: 1) What if the Bonus Round player becomes sick and is unable to continue? 2) If someone becomes sick during the taping and is unable to continue, is said player rendered ineligible?

I also could have sworn I have a vague memory of a WoF episode when I was real little where a different person was in the red podium between the 3rd and 4th round, but I don't remember any details. Just a vague (if real) memory. I would think their lawyers have a procedure in place since while not a great probability, is possible to happen. woffan2008, there was a situation where Pat lost his voice before the bonus round so Vanna and him switched spots where he turned the letters. Price let that girl keep the 30k from Plinko off-air when they kept the rigged board (set up for an advertisement). I would imagine the same here.

Closest I can think of is the one celeb game where William Shatner had to leave mid-game. Someone asked Pat about that on Twitter, and he said it was because they were having a ton of technical difficulties that made the taping run really long, thus creating scheduling conflicts.

I have a vague memory of attending my first taping at the Palace of Fine Arts in San Francisco for a Celebrity week in 1996--I believe there was an instance where mid-game they changed celebrity players...or maybe it was a different episode. I can't remember!

They didn't have a Celebrity Week during that remote. It was just a regular week and a College Week. Perhaps you were thinking of that or the Soap Opera College Challenge week taped in 1992 at that same location?
