All right! At long last, I found my old copy of World's Dumbest Crooks and Other True Tales of Bloopers, Botches and Blunders by Allan Zullo. This was something I wanted to bring up here for the longest time; I can't remember if I did already.
In the chapter "Gollywood Goofs," there is a story titled "Vanna White's Most Embarassing Moment," albeit with the vowels unrevealed. I'm not going to type out the story word for word, but here's what happened.
During a taping, which presumably occurred while Vanna was still turning letters, the zipper on the back of the strapless dress she was wearing ripped open. Thankfully, she was able to grab the front of her dress before it fell. She ran offstage and returned moments later wearing a new dress. Needless to say, the audience members were on their feet applauding when she returned.
Has anyone else heard about this moment?
BTW, the book also mentions some moments on TPIR. One moment involved a car accidentally being pushed through a wall on the set, although the model on top of it kept smiling and waving as if nothing as happening. Another moment mentioned in the book was probably one made a clip in the 25th Anniversary Special in 1996. It involved a playing of Master Key where the key got stuck. A third moment was when a woman chased Barker all over the set after winning a prize. There is not much detail besides that, but this was probably after the first ever $10,000 win of Grand Game. Now, I thought I heard that occurred on the first-ever playing of that game. Is that correct?

Pauline's victory (the first full $10,000 victory) was on the eighth playing of the game.
