If it's okay with the mods, I'll tell you what it is without the first word of the puzzle.

Has THE THRILL OF VICTORY AND THE AGONY OF DEFEAT been used more than once?

Thankfully that didn't result in a big blunder; in fact Casey solved it rather quickly. Also, the first part (THE THRILL OF VICTORY) was used in season 28 (Becky messed that one up with THROES).

^ That sounds like something for the DPG.
DESE DEM AND DOSE GUYS has to get a nod in my opinion.

How about... BARBECUE SPIT? The P was missing and a contestant called...out of all letters, an H!

I enjoyed "CARIBBEAN QUEEN BY BIFFY OCEAN!" by a contestant when she was on Free Play. The best part was that as she spun again, she said something like, "I don't know who Biffy is, but..." Ha!

biffyocean - Looks like that episode got a mention! I forgot about DESE DEM AND DOSE GUYS. The first word rhymes with "These", but the contestant pronounced it as if it rhymed with "Jessie". Twice. And then... - "Listen, he's got all the letters up; I'm takin' it!" - PatFor a premiere episode, Season 13's Round 1 puzzle of RHYTHM GUITAR was a toughie. The final scores were very low, although the winner also solved the bonus puzzle for $25,000. The Season 16 Philadelphia week had a bonus puzzle of THAT'S IFFY, which itself was booed by the audience, following its reveal after time ran out. I think the booing would have been notable back in first run.

^ I'm guessing he's referring to NEW YORK AND DAMN YANKEES. Sorry for not replying sooner, but that's the one. New York Yankees was the second part of the puzzle.

I think the booing would have been notable back in first run. I seem to remember a couple other bonus puzzles getting booed. I think one of them was QUIZ SHOW back in Season 6.

How about... BARBECUE SPIT? The P was missing and a contestant called...out of all letters, an H! Same thing happened a couple weeks ago. I forget the exact puzzle, but it was the Prize Puzzle that ended in BATHING SUIT. Not sure anyone noticed, though.

A Before & After of "NANCY GRACE PERIOD" from Season 26 made the rounds at the time because it sounds so wrong. Also around the same time, "MARCO POLO SHIRT", which went viral for the several seconds where the puzzle's state was "_A___ ____ SHI_T". Pat sounded annoyed when one contestant tried to fill in that notorious blank with an F, then said "Thank you" when the next contestant called R.
